To move you into wellness and prevention. To ultimately improve your overall health, not simply treat a symptom.

If you are suffering from chronic pain, we will work with you to relieve the pain. But we will also see if we can find a way to prevent it from returning.

If you are having difficulties with speech or swallowing, we have specific therapies to improve your ability to perform these functions. But we will also strive to keep those improvements in place.

We will work with you to gain your independence whether it means driving a car again, having better balance when walking and moving, or being able to function independently within your own home.

We will look at your condition holistically, taking into account not only your physical condition but other factors that might be influencing your condition such as stress, lifestyle or environment.

We have a responsive, dedicated outpatient team that understands that schedules change, and needs often crop up suddenly.

Care and concern are just as much a part of the outpatient program as they are in the hospital.

Each program is comprehensive.

And designed to meet each patient's particular needs.

When you are referred to us, you become part of a dedicated rehabilitation team that aims at gaining you as much recovery and independence as possible. Each patient has a primary therapist. Other professionals become part of the team as needed. They may include an occupational therapist, physical therapist, speech language pathologist, medical social worker, or psychologist.

Southwest Rehab is well known for its interdisciplinary, team approach to rehabilitating patients. As an outpatient you should expect the same team approach to meeting your needs as if you were a patient in the hospital.

Referral to Southwest Regional Rehabilitation Center is easy. Your physician simply needs to make a phone call. Once the call has been made, Southwest Regional Rehabilitation Center will provide an evaluation within 24 hours. A program will be designed to address your needs. All rehabilitation programs at Southwest Regional Rehabilitation Center are overseen by a staff case manager.

The purpose of the program is simple. It's to help you be more independent. It might mean getting you back to work. Or letting you work in your garden. Making you feel comfortable in your own home again. Walking through your own neighborhood.

To accomplish that might involve working outside the hospital environment. Therapists may need to come to your home, your backyard, work with you in the places where you want to get back to normal life.
