Rehabilitation Process
Southwest Regional Rehabilitation Center provides a comprehensive rehabilitation program for persons who have medical conditions that impact their ability to gain strength, mobility and endurance. Our services range from intensive inpatient treatment to outpatient programs, which assist in improving and maintaining maximal functional capacity. Our goal is to return the individual to the community quickly, with the highest functional level possible. The Decondition Program is designed to serve patients recovering from many types of conditions, which have produced functional limitations. Examples include, but are not limited to, pulmonary conditions, cardiac conditions, and post-operative surgically-related primary or secondary conditions.
Inpatient Rehabilitation
Once admitted to the inpatient rehabilitation program, treatment begins with a comprehensive evaluation to identify the patient's functional status. The rehabilitation team then works together with the patient and family to establish treatment goals. Each patient's program is tailored to meet his/her individual needs and is modified as progress occurs. Our Medical Director and physiatrist, who collectively specialize in internal and physical rehabilitative medicine, direct the treatment team. Other team members include: physical and occupational therapists, rehabilitation nurses, a speech pathologist, social worker, and the patient and family.
The inpatient program provides 24-hour-a-day physician and nursing support to address the medical and nursing needs of the patient. Individual and group therapy sessions are conducted for a minimum of 3 hours a day.
Specialized treatment and services offered include:
- Dynamic balance training program with emphasis on fall prevention
- Restoration of functional mobility with emphasis on safety and appropriate biomechanics
- Treatment for movement disorders and gait dysfunction
- Strength, flexibility, and range of motion programs
- Identifying joint dysfunction and improving joint integrity and flexibility
- Cardiac rehabilitation with focus on energy conservation and building endurance
- Activities of daily living training (feeding, dressing, grooming)
- Home evaluations and recommendations for home safety
- Determining need for durable medical equipment
- Expressive and receptive language treatment
- Swallowing evaluation and treatment
- Rehabilitation psychology
Participation by family and/or significant others in the rehabilitation process is strongly encouraged for patient success at discharge. All team members are involved in patient and family education. As discharge approaches, patients and families are counseled regarding the resources available to help make a smooth transition to home or to another care giving setting. Arrangements are made for those who would benefit from ongoing therapy at home or as an outpatient.

Comprehensive Outpatient Rehabilitation
The outpatient program is designed for the individual who is able to live in the community, but requires ongoing physical, occupational, or speech therapy. The rehabilitation team provides coordinated rehabilitation services including assessments, treatments and follow-up care.
Outpatient Services include:
- Full range of physical and occupational therapies
- Speech language pathology
- Aquatic therapy
- Wheelchair seating and assessment for proper fit, comfort and mobility
- Comprehensive orthotics services
- Rehabilitation psychology
- Driver rehab evaluation and treatment
- Social work care coordination services
Outpatient therapy services are scheduled according to the patient's needs, generally two to three visits per week. Information regarding transportation assistance is offered through outpatient registration for those needing assistance to and from therapy.
