Admission, Discharge, and Referral - Inpatient Admissions

To begin the inpatient admission process at Southwest Regional Rehabilitation Center, call our case manager at 269-274-2691. This puts you in touch with an admitting representative who can assist you with general information. To facilitate inpatient admissions, an admitting case manager conducts a telephone assessment and typically an on-site evaluation of the prospective patient to identify their rehabilitation needs.

The case manager collaborates with Southwest Regional Rehabilitation Center's Medical Director in determining the status for admission. Southwest Regional Rehabilitation Center accepts referrals from physicians, social workers, nurses, therapists, discharge planners, insurers, case managers, patients themselves, family members, and friends.

For admission to the hospital to be considered appropriate, the following criteria should apply:

To benefit from inpatient rehabilitation it is required that the patient have a reasonable expectation for significant improved function as a result of the rehabilitation program which may include the following:

To maximize the rehabilitation process and to complete the inpatient criteria, the patient should have an appropriate discharge plan, and be willing and able to participate in the rehabilitation program.

Southwest Regional Rehabilitation Center, with our current facility and program scope, is not able to provide clinical support for patients with the following conditions:

Patients who fall into any of these categories will be assisted by either the hospital's case manager and/or medical social worker in identification of medical rehabilitation resources which can support their clinical management.

Southwest Regional Rehabilitation Center offers comprehensive therapy services for all age groups. It is our goal to assist patients in attaining the highest functional level possible and to enhance their quality of life through the rehabilitation process.

Discharge and Community Reentry

We want your discharge to be smooth and comfortable. The Medical Social Worker will work with you to make the necessary arrangements for any additional services you may need. You and your family will be actively included in this process. We don't want you to leave Southwest unprepared.

Insurance

Prior to admission, we work with your insurance company to verify coverage for your hospital stay. Once admitted and throughout your stay, we work with your insurance company to keep them apprised of your goals and progress to assure continued coverage. Should you require additional services after discharge; the Medical Social Worker will advise you of coverage options.

Follow Up

The relationships that you develop with your Rehab team during your inpatient stay don't always end when you're discharged. To reach your fullest recovery, you may require outpatient services, such as follow-up appointments with your physician and sessions with your therapists. (You may choose an outpatient service provider that is nearer your home, as well. ) We will contact you 2-3 days after your discharge as part of our follow-up program. We want to know how you are doing.
